<p style="text-align: justify;">April 17, 2012 &#8211; The Israel-U.S. Binational Industrial Research and Development (BIRD) Foundation has approved an investment in a joint development project between MedCPU and the Cleveland Clinic to develop a care pathways decision support system. The BIRD Foundation promotes collaboration between Israeli and American companies for the purpose of joint product development. The BIRD Foundation works in full cooperation with the Chief Scientist&#8217;s Office at the Ministry of Industry, Trade, and Labor in Israel, and with the U.S. Commerce Department&#8217;s National Institute of Standards and Technology (NIST).</p>

<p style="text-align: justify;">Eyal Ephrat and Sonia Ben-Yehuda saw an opportunity to develop a technology that reads and analyzes notes written by physicians in electronic medical records.</p>

<p style="text-align: justify;">As a result, they founded MedCPU Inc. in 2008 to sell the computerized clinical decision-making tool to hospitals.</p>
<p style="text-align: justify;">&#8220;They recognized that over half of the data, which is often the most meaningful, is unstructured data (narrative notes) from clinicians about the patient, and they wanted to capture that unstructured data,&#8221; said Dan Neuwirth, president and CEO of MedCPU Americas, the company&#8217;s U.S. operating unit that is based in Henrico County.</p>
<p style="text-align: justify;">The company&#8217;s MedCPU Advisor is a user interface visible on top of electronic medical records in hospitals.</p>
<p style="text-align: justify;">The Advisor reads structured electronic medical record entries and unstructured notes. It compares the information against knowledge-based protocols and guidelines to advise physicians of any deviations from clinical best practices.</p>
<p style="text-align: justify;">&#8220;We can capture a full clinical picture of the patient,&#8221; Neuwirth said. &#8220;If you only use the structured fields, you can&#8217;t do that.&#8221;</p>
<p style="text-align: justify;">The tool ensures the highest level of clinical care.</p>
<p style="text-align: justify;">&#8220;It also streamlines the workflow of a clinician,&#8221; Neuwirth said.</p>
<p style="text-align: justify;">&#8220;From a technology perspective, they have a real innovation,&#8221; said Michael Pirron, CEO and founder of Impact Makers, a Richmond-based information technology and management consulting company focused on health care.</p>
<p style="text-align: justify;">&#8220;They can pull data from free text in electronic medical records and use the data to improve clinical outcomes. That is exciting to see,&#8221; Pirron said.</p>
<p style="text-align: justify;">MedCPU&#8217;s founders started the company in New York but moved the corporate offices to Henrico last year when Neuwirth joined the firm.</p>
<p style="text-align: justify;">This year, MedCPU will move its offices to the Turning Basin building in downtown Richmond, where several biotech companies are located.</p>
<p style="text-align: justify;">&#8220;We want to be part of that community,&#8221; Neuwirth said.</p>
<p style="text-align: justify;">Before joining MedCPU, Neuwirth had worked for GE Healthcare&#8217;s Performance Solutions business after its 2008 acquisition of Henrico-based Agility Healthcare Solutions, which he co-founded and served as its chief operating officer. Before that, he had worked for pharmaceutical distribution and health care technology firm McKesson.</p>
<p style="text-align: justify;">MedCPU targets hospitals and hospital systems across the country as well as a larger delivery network that includes hospitals and ambulatory locations. The company has clients nationwide but none currently in Virginia.</p>
<p style="text-align: justify;">&#8220;There is a lot of energy here already,&#8221; Neuwirth said. &#8220;Our clients tend to be leading academic medical institutions and larger hospital systems, partly because they are further evolved in (electronic medical records) implementations.&#8221;</p>
<p style="text-align: justify;">Revenue increased 800 percent from 2010 to 2011. The company hopes to increase sales as health care providers begin to comply with the &#8220;meaningful use&#8221; of the health care technology section of the American Recovery and Reinvestment Act of 2009.</p>
<p style="text-align: justify;">&#8220;Our product is certified by the government to support the meaningful use requirements,&#8221; Neuwirth said.</p>
<p style="text-align: justify;">Health-care providers using systems of certified technology are qualified to receive federal stimulus money upon demonstrating meaningful use of the technology. The meaningful use criteria will be staged in three steps through 2015.</p>
<p style="text-align: justify;">Impact Makers&#8217; Pirron has worked with MedCPU&#8217;s offices in Henrico and in Israel and has enjoyed being involved with the company.</p>
<p style="text-align: justify;">&#8220;They have been wonderful,&#8221; he said. &#8220;It&#8217;s a great culture to work with. It&#8217;s moving and growing at one thousand miles a minute.&#8221;</p>
<p style="text-align: justify;">MedCPU opened its research and development office in Tel Aviv, Israel, in 2008. The company took the first two years to develop the product.</p>
<p style="text-align: justify;">&#8220;We have clinicians in Israel that are software developers, which gives us a competitive advantage,&#8221; Neuwirth said.</p>
<p style="text-align: justify;">Tonya Signa, president of Signature Marketing Services, the Phoenix-based marketing company that has worked with MedCPU, finds MedCPU to be very collaborative.</p>
<p style="text-align: justify;">&#8220;MedCPU has made strategic investments,&#8221; she said. &#8220;They are thinking about the future and developing technology that will lower the cost of health care.&#8221;</p>

<p style="text-align: justify;">The adoption of technology, more specifically EMRs and clinical decision support systems, is critical for reducing healthcare costs, improving quality and supporting clinicians in their increasingly complex roles. It’s naive to think, however, that we will achieve these outcomes by adding more technology and more tasks to the clinician’s work flow. Technology adoption cannot happen at the expense of providing top-notch clinical care. After all, one of the tenets of medicine is “first do no harm.”</p>
<p style="text-align: justify;">Clinicians do need support when and where it matters most — the exam room. Clinicians need simple, clinically smart solutions that capture the complete clinical picture in real time without changing or adding to the work flow. Every added piece of technology needs to work in harmony to enhance patient care and improve outcomes.</p>
<p style="text-align: justify;">Two related issues driving physician pushback of EMR adoption include usability concerns and work flow disruption. Time spent looking at a computer is time not spent engaging a patient face to face. The <em>Journal of General Internal Medicine</em> article “<a title="PubMed summary" href="http://www.ncbi.nlm.nih.gov/pubmed/19130148?dopt=Abstract" target="_blank">Primary care physicians’ use of an electronic medical record system: a cognitive task analysis</a>” reported that using EMRs increases physicians’ average screen gaze time by more than 100%, rising from 25% to 55% of consultation time, inevitably resulting in less eye contact and less meaningful conversation with the patient.</p>
<p style="text-align: justify;">Clinicians have long-established work flows that often include documenting patient encounters in free text. While EMRs allow free-text notes, only the data residing in structured fields is available for analysis. The way for clinical decision-support tools to be effective is by bringing alerts to physicians and providers in their existing course of action at the point of care, or when they enter both structured and free-text notes. By receiving prompts while the clinician is with the patient in the exam room, physicians and other healthcare providers may alter treatments and courses of care to reduce variances and drive improved outcomes.</p>
<p style="text-align: justify;">A <em>New England Journal of Medicine</em> column, titled “<a title="New England Journal of Medicine" href="http://www.nejm.org/doi/full/10.1056/NEJMp0802221" target="_blank">Off the Record — Avoiding the Pitfalls of Going Electronic</a>,” warns that structured text fields, namely drop-down menus and discrete data fields lead physicians “to ask restrictive questions rather than engaging in a narrative-based, open-ended dialogue,” which can be “key to making the correct diagnosis and to understanding which treatment best fits a patient’s beliefs and needs.”</p>
<p style="text-align: justify;">The same is true of a robust clinical decision support tool. To be truly effective, a guidance tool must offer prompts based on the complete clinical picture of a patient, comprised of information recorded by a physician’s structured and narrative notes.</p>

There’s no question that healthcare needs to adopt new technology that makes us more effective and efficient and curbs costs, like Electronic Medical Records (EMR) solutions and Clinical Decision Support (CDS) systems. In today’s world, providers of all sizes continue to find it challenging to integrate existing HIT systems with EMRs for a variety of reasons. As our industry evolves, technology solutions need to be smarter and empower seamless integration.</p>
<p style="text-align: justify;">EMR and HIPAA guest author Susan White covers in depth how <a href="http://www.emrandehr.com/2010/08/05/guest-post-medical-devices-and-emr-integration-%E2%80%93-a-more-convenient-future-or-troubles-galore/">a lack of connectivity standards affects EMR integration</a>. There are no mandated standards for EMR vendors to follow, making it hard to coordinate data sharing between medical devices and other systems (including from one EMR to another), even at the same facility. As those systems operate in disparate fashions, critical clinical information is often lost or stuck in silos. Most importantly, the information is not where clinicians need it most–at their fingertips, in an exam room, with a patient.</p>
<p style="text-align: justify;">This lack of data sharing is a pervasive concern. One <a href="http://www.markle.org/sites/default/files/20110110_HINLSurveyBrief_1.pdf">Markle report</a> finds that roughly 80 percent of both consumers and physicians demand that hospitals and doctors be required to share information that improves coordination of care, cuts unnecessary costs, and reduces medical errors.</p>
<p style="text-align: justify;">In 2010, more than $88 Billion were spent on developing and implementing EHRs, health information exchanges (HIEs) and other health IT initiatives. When you consider that the average 10-physician practice spends more than <a href="http://www.aafp.org/online/en/home/publications/news/news-now/practice-management/20090804hasc-rpt.html%29">$137,000 per year</a> on prior authorizations and pharmacy callbacks alone, you’ll have to agree that the lack of data integration and sharing get very costly. And although I agree with John Halamka, who recently wrote these challenges exist because <a href="http://geekdoctor.blogspot.com/2011/08/healthcare-is-different.html">healthcare is inherently more complicated</a> than other industries, I am a strong believer that a lot of them can be overcome by the use of smart technology.</p>
<p style="text-align: justify;">We need smart, flexible solutions, which capitalize on existing technologies and require minimal integration. Technologies that employ advanced screen extraction, for example, empower several important improvements in the clinical decision support space such as the capturing and analysis of both free and structured text. A lot of time such solutions are rendered ineffective as they either lack compatibility with leading EMR systems or are too hard to integrate.</p>
<p style="text-align: justify;">As the industry evolves, developing robust protocols for capturing both structured and unstructured data along with standards for data integration and sharing will become increasingly important. With all the data points created on patients every day, we will need a consistent, secure, and reliable way to capture and share patient data among all systems and healthcare providers.
